Originally, he only kept his hair in spikes, jagged dark spears that seemed to cut the air with as much savagery as his unnamed blade. He had both eyes then, and he killed more easily than he breathed.
It was so easy it wasn't fun.
The eyepatch came first, and he only blames Yachiru a little for laughing when he walked into immovable objects. Not that it hurt, but that it was shameful to be less than his usual tightly coiled spring of homicidal rage.
Originally, the bells were a trick she played on him, putting the bells in his hair while he wasn't paying attention, and he later found that they made no difference in his ability to coat his Soul Slayer red with blood. All the bells did was amuse Yachiru and frighten his enemies.
He keeps them for one of those reasons and does not say which.
